problem solved! :mrgreen:
replaced the south bridge of 12xx board fix the problem for me.
so it seems the problem(don't sure about all cases) is coming while the game try to read(maybe write) save file from/to the hdd.
so I thought it can be related to sata controller.
on revisions 12xx (i'm pretty sure on slims also) the sata traces connecting directly to the south bridge.
on the faulty south bridge I get around 14k ohm between each of two caps against GND, after replace got around 10k ohm and problem gone completely (I marked the points in red ,sorry of the poor pic I suck on these stuff)

costumer come back to me again with the same problem after he play 1 week without the error :facepalm:
gonna check this console again next week.

Good luck, this has been a pain for me too. Whenever Ive replaced the south bridge to fix HDD issues on the 12XX series it is like a hit or miss. I havent been able to pinpoint the problem beyond that. The SAX-001 boards were much better with their independent controller. Too much reliance on 1 IC on the newer systems.

i too have the same issue i replaced the southbridge still error comes back i think need to replace thoes caps or could be bad sectors on the hdd going to run a sector scan on the hdd
